Please make sure all fields are filed out. Then put it in the yellow envelope along with your old Ghost Controls control board placed in smaller white box. You will need to place the return label on the yellow envelope and place your old control board in the white box the other control board came in. Place your printed rebate form in the envelope, seal and place in your mail box or drop off at any USPS location near you.
